用人单位携带营业执照、批准成立证件或其他核准执业证件、组织机构统一代码证书原件及复印件、填写“社会保险登记表”一式两份申请办理社会保险登记,审核通过的,发给《社会保险登记证》。
变更、注销登记的,用人单位携带有关机关批准变(或宣布)变更证明,填写“社会保险变更登记表”或“社会保险注销登记表”,审核通过后,重新核发或缴销《社会保险登记证》。
Employers should bring their business license, establishment approval certificate or other practice approval certificate, original and copy of the organization code certificate, and fill out two copies of the "Social Insurance Registration Form" to apply for social insurance registration. If approved, a "Social Insurance Registration Certificate" will be issued.
For changes or cancellations of registration, employers should bring the change approval (or announcement) certificate from the relevant authorities, fill out the "Social Insurance Change Registration Form" or "Social Insurance Cancellation Registration Form", and after approval, the "Social Insurance Registration Certificate" will be reissued or cancelled.
